This service allows customers to apply for a license  a community or compounding pharmacy, whether independent or within a health facility.for a new facility.

• Initial approval (owner profile check & primary inspection):

  • A copy of the owner's valid passport

  • A copy of the owner's family book

  • A copy of a valid ID card

  • Recent color photographs

  • An introductory statement filled out exclusively by the owner and the responsible pharmacist (the statement is available at Customer Happiness Centers)

  • The site’s architectural drawing approved by an engineering consultant office

  • Location photos

• License initial approval for new facility:

  • Affection plan attested by the municipality

  • A copy of the establishment lease agreement

  • A copy of the trade name reservation

• Final inspection and final approval of the license for new facility:

  • A valid copy of the trade license issued by the Department of Economic Development

  • A copy of the license of the pharmacist responsible for the pharmaceutical facility

  • A valid copy of the certificate of conformity with preventive safety requirements issued by Civil Defense

  • A list of partners
