About the Service

The service enables customers to request the issue of an official certificate upon the death of a person. The death certificate will state the date, place and cause of death as well as the nationality and religion of the deceased. The original documents of the deceased should be submitted.

Service Process

    When applying through the website:

    • Sign in to e-service portal, obtain username and password, and login to e-system.

    • Submit the application and pay the fees.

    • Attach the relevant documents.

    • Public Health Center in the respected emirate will verify the attachments.

    • Receiving the certificate by delivery to the person concerned or a representative or by visit a public health center in the same emirate for verification and matching of original documents.

    When applying through public health center or hospital:

    • Submit the application through the e-system and pay the fee.

    • Once the documents and original copies are verified, the birth certificate.

    • Receipt of the certificate by the deceased's family or a representative.

Conditions & Requirements

  1. Attested death notification.

  2. Original documents / copy of the original documents.

  3. Name in the police statement must match the name in passport or Emirates ID card.

  4. Visit the public health center in the emirate where the death took place, with the required documents.

  5. Death certificate issued and attested from Department of Health Abu Dhabi, or Dubai Health Authority, or Emirates Health Services is equivalent to that of MOHAP attestation.

  6. Death certificate should be attested through the UAE Ministry of Foreign Affairs and International Cooperation as well as the Embassy of the country in which the death certificate will be used.

  7. A forensic report indicating the cause of death must be submitted in the case of criminal suspicion or when the cause of death is recorded as unknown in the death report.

  8. The death certificate will be presented to the parties or authorized persons concerned, or those specifically stated in a letter from the Consulate, a letter from the deceased's place of work, or a letter from the police..

FAQ’s

  1. The death certificate stamped by the Department of Health in Abu Dhabi or Dubai Health Authority or Emirates Health Services is equivalent to the stamp of the MOHAP. The certificate might need attestation from Ministry of Exterior and the embassies.
  2. The certificate fee is AED 65 Arabic and AED 65 English certificate (if requested).

Death certificates attested by Department of Health Abu Dhabi or Dubai Health Authority or Emirates Health Services are equivalent to those of the Ministry of Health and Prevention.

Required Documents
  • If death occurred in the UAE:

    • Registration by the application through e-services on the website, at a public health center, or the hospital.

    • Copy of Emirates ID or passport, along with original copy.

    • Deceased nationals: copy of original family book.

    • Deceased expats: copy of passport and original with Visa and/or copy and original of UAE ID.

    • Electronic death notification from hospital according to the approved form.

    • Statement from hospital police office or from district police station if death occurred at home.

    • Forensic medical report in the case of criminal suspicion.

  • If death of a UAE national occurred outside the country:

    • A death certificate certified and translated into Arabic by the UAE Embassy in the country in which death occurred and a report from the UAE Ministry of Foreign Affairs and International Cooperation stating the cause of death.

    • Death certificate duly attested by the country in which the death occurred.

    • Family book of the deceased and original passport.

    • Certified statement or proof of deceased's burial outside of the country or within the UAE.

    • Certificate will be provided to the deceased’s family or other delegated person.
